Short-acting: Oxycodone (Roxicodone) or in acetaminophen and oxycodone (Percocet, Roxicet)
Dosing: start 5-15 mg q 4-6 h in opioid-naive patients
With all combinations, doses limited by maximal daily dose of the nonopioid component (e.g., 4 g/d of acetaminophen)
Extended Release: Oxycodone (OxyContin) (contains no nonopioid additive)
Dosing: 10, 15, 20, 30, 40, 60, 80 ER q 12 h
Individualize dose based on current short-acting opioid intake. May increase q 1-2 days.
Do not use Extended Release with opioid-naive patients.
With regular daily dosing, strongly consider Senna laxative for constipation (not bulk products). Generally, constipation is a chronic effect of opioids
