DRUG TREATMENT - Opioids - General Info Opioid Conversion for Equianalgesic Approximation
Drug cross tolerances can vary by as much as 25-50%. This means that if a patient develops tolerance to one opioid, a significant amount of that tolerance may dissipate in the face of another opioid that the patient has been switched to and lead to increased respiratory depression and increased analgesic effect at reported equianalgesic doses.
A quick conversion table, courtesy of the CDC, is located below as well as an excellent and simple opioid conversion calculator. Dr. Fudin got his fame at the VA.
